With the Mybusiness form to database widget you can easily create a form taht all information that will be filed into it will automatically be sent to your table of choice.
You will have full control over your form and will be able to choose which fields from your table you want to include in your form.
Step 1:
Drag & drop the "form to database" widget, fill in a form name and success/failure message or link to redirect too after sending the form.
Step 2:
Choose your table you want to send the data to and choose which fields you want to add to the form.
Step 3:
Double click an input line to edit the name label and other options.
